- Managing your profile - When you create a profile, we require some information from you to make it happen. Consider your e-mail address, name and address details, gender and date of birth. This data enables you to use certain services on our website and digital patient registration system. By creating a profile, you don't have to fill in the information over and over again.
- Registering and making an appointment - If you register with Loomans Osteopathy or make an appointment via email, the information you provide will be used solely to manage your registration and make an appointment.
- Anamnesis form - If you complete the anamnesis form through our digital patient registration system, the information you provide there provides us with a general understanding of your health, medication use, conditions and allergies. We treat the information you enter as confidential and use it only to provide you with appropriate care.
- Your medical record - In the event you provide medical information to Loomans Osteopathy, it may be included in your medical record. Loomans Osteopathy does so only if legally permitted or we have obtained your express consent to do so.
- Contact form - If you have any questions you can always contact Loomans Osteopathy by filling out the contact form. Asked to enter your name and contact information along with a description of your question. This information is used exclusively to handle your inquiry and to improve our services.
